Outpatient Appointment

 

Thank you for choosing to see me at the Nuffield Bristol Hospital – The Chesterfield for your outpatient consultation.  This letter sets out some important information that I am required by law to provide to you.  This is for your information only and is not a bill.  As this includes information about my charges, if you do not have private medical insurance but someone else will be paying your bill, you may wish to pass a copy of this letter to them.  Please note that even if someone else is paying your bill or you have private medical insurance, you are responsible for paying any charges which they do not pay. 

 

Consultation Fee

 

My fee for an initial consultation will not exceed £<see fees page or your individual letter for latest> and my fee for any follow-up consultation will not exceed £<see fees page or your individual letter for latest>.  These estimates are correct as at the date of this letter. Should you cancel your appointment with less than 24 hours’ notice, a cancellation fee of up to 50% of the appointment charge may be charged.

 

Following your consultation you may need certain tests (such as blood tests or imaging, for example an X-ray, MRI or CT scan) to help me diagnose your condition.  If the test is undertaken by the clinic or hospital, and not by me, the fees for those tests will be determined by the clinic or hospital and charged to you, or your private medical insurer, separately.  

 

If there are any fees which I will charge in relation to any of the tests I advise that you have, I will let you know what those will be.

 

Private Medical Insurance

 

We are not currently working with private medical insurers unless we have arranged this with you separately beforehand.

 

Financial Interests

 

I am legally obliged to tell you if I have any financial interests in The Nuffield Bristol Hospital – The Chesterfield or any equipment there.  I can confirm I do not have any such financial interests.

ABOUT US:

Adam Bray Dermatology Ltd (Registered in England No. 8325506, Registered

Address c/o Andow Ellis Ltd, Trym Lodge, 1 Henbury Road, Westbury-On-Trym, Bristol. BS9 3HQ). Dr Adam Bray is the consultant who provides the medical services. We will try to accommodate your appointment request but if we cannot do so we will try to inform you of colleagues you may wish to approach instead, and we will recommend someone where possible.

 

OUR SERVICES:

Adam Bray Dermatology Ltd offers private dermatology care. As such, a recommendation for an intervention or other treatment will not be given if this is contrary to the expert opinion of the consultant. The terms ‘We’ or ‘Us’ used below refer to Adam Bray Dermatology Ltd. ‘You’ refers to any patient who has contacted or registered with Adam Bray Dermatology Ltd to obtain a quotation for, to arrange, or to receive, medical advice or treatment.

 

 

CONTRACT:

We will provide private medical services, as defined under the ‘Our Services’ section above, and within the scope of our clinical practice, to ‘You’, the patient. By arranging a consultation or treatment with us you are entering into a contract to agree to pay us the fees for the agreed medical services.

 

LIABILITY:

Adam Bray Dermatology Ltd will not be liable in contract, tort or otherwise for any economic loss (including, without limitation, loss of profit), or for any other special, indirect or consequential loss or damage arising out of, or in connection with, its provision

of any goods and/or services to you.

 

It is your responsibility to ensure that you provide us with all relevant medical details at

each consultation or treatment. Relevant medical details may include the following (but this list is not exhaustive:

  • A referral letter from another doctor (usually your GP) to Dr Bray
  • Important test results such as a skin biopsy
  • Any previous or current medical conditions you have suffered or diagnoses you have been given
  • Any current medications (including supplements, vitamins, herbs or other remedies, contraception, inhalers, implants) 
  • Any known or suspected allergies, sensitivities, or adverse reactions (especially to medications or latex)
  • Any previous operations, especially any complications, wound infections, or reactions to anaesthetic 
  • Medical problems that affect close family members (e.g. Conditions that ‘run in the family’ or are inherited so usually affect several family members)

 

CANCELLING AN APPOINTMENT: 

If you are unable to attend your appointment, please contact us immediately by email at the address above. We will need to know that you wish to cancel by 24 hours before your appointment is due. Failure to cancel or rearrange before this time will result in you being liable for up to 50% of your consultation fee.

 

If a surgical procedure is cancelled without a medical reason within 48 hours, you will be liable for 50% of the fee.

 

  • Please arrive for your appointment 5 minutes in advance so that the necessary paperwork can be completed. Late arrival may result in reduced consultation time or forfeiting of the appointment. Adam Bray Dermatology Ltd will endeavour to ensure that your appointment runs to time. However every patient has different needs and we also wish to ensure that each patient has the consultation time they require.  We may also try to fit in additional tests or procedures on the same day as your consultation for your convenience if required. Should we need to cancel or postpone your appointment at short notice, we will make every effort to contact you in advance.

 

REFERRER/GP:

It is usual medical practice for us to inform your GP (and/or other healthcare provider referring you to us) by sending them a letter summarising your consultation and the plan for treatment. If you do not wish this to happen then you must let us know at or immediately after your consultation or treatment.

 

SELF-FUNDING PATIENTS:

You agree to pay for the services that we have provided to you, as per the fee schedule detailed above. For consultations payments should be made in advance by bank transfer – details will be sent. 

 

For procedures, the fees charged by us are exclusive to any additional charges that may be made by the hospital. Procedure fees are payable by direct bank transfer 7 days before your operation date. A cheque may be accepted but this will need to clear 7 days before your operation date.

 

Prices for other tests, surgery or services which are not listed will be quoted by Adam Bray Dermatology Ltd and The Nuffield Hospital Bristol – The Chesterfield in advance, following consultation.

 

INSURED PATIENTS:

We will invoice your insurance company directly if you provide us with complete details of your scheme, policy number and pre-authorisation number. Please be advised that insured patients are liable for full payment as the contract to provide services is with you the patient, and not the insurance company. You must therefore inform your insurance company of consultations and surgery which you organise with us and confirm you have adequate cover. If your insurer declines to settle your invoice in part or in full, then any shortfall or lack of cover will be due from you. If you are unsure we recommend you contact your insurer for advice before scheduling any services with us. We will adhere to the fee schedules set by the major insurance companies and details may be obtained directly from the individual insurance company. These fees differ to the fee schedule which we have for self-funding patients. 

 

SPONSORED PATIENTS:

A sponsored patient is required to settle their account on the day of the appointment, as per the arrangements for a self-funding patient, by debit or credit card. A guarantee of payment or any other payment is not accepted.

 

LATE OR RETURNED PAYMENTS:

Interest on overdue invoices is applied at a rate of 8% a year plus the reference rate and is applicable once 14 days have elapsed following consultation or treatment. Adam Bray Dermatology Ltd reserves the right to charge a £10 administration fee for late or returned payments in addition to reasonable debt recovery costs incurred.

 

UNPAID EXCESSES:

Insured patients will be liable for any excess payment as determined by their private medical insurance contract. This will be clearly communicated by your insurer and is normally paid directly to the provider (Adam Bray Dermatology Ltd). We reserve the right NOT to provide further medical services in the event of any late or outstanding excess payments to us. If you are in the middle of a treatment pathway then onward referral, or referral back to your GP, will be arranged.

 

REFUNDS:

Refunds are not provided.

 

FRAUD AND DEBT COLLECTION:

Information may be disclosed to others, including debt collection agencies, with a view to recovering any unpaid debts or other costs. Where we instruct debt collection agencies, you may become liable for any additional costs.

 

DATA PROTECTION & INFORMATION GOVERNANCE:

Please be aware that any information you send by email is sent 'in the clear' unless specifically encrypted by you. This means the information could be intercepted and read by an unauthorised intermediary. If the information is sensitive then please encrypt your message (your existing email application may offer this capability or use your search engine to find one). If you send an encrypted message you will receive an encrypted reply, otherwise the reply will be sent in the clear. Alternatively please request a telephone call back and leave your contact number.

 

Any personal data that we record during your consultation will be held securely encrypted. We will ask your consent to share any of this data with you by email for your convenience (e.g. a summary of your consultation or copies of photographs). These can be sent via encrypted PDF attached to normal email or via fully encrypted email if you prefer. No personal data will be shared with any other person or body without your explicit consent except for reasons of managing your direct medical care (e.g. your GP or The Nuffield Hospital or onward referral to another specialist by prior agreement with you).

 

ARTIFICIAL INTELLIGENCE FOR DIAGNOSIS AND TREATMENT: 

For the avoidance of doubt, we do not use artificial intelligence for diagnosis or treatment decisions.

 

ARTIFICIAL INTELLIGENCE FOR ADMINISTRATION:

We are using artifical intelligence to streamline administrative tasks. Voice to text digital dictation is used to prepare clinical records and correspondence, to reduce the need for typing. We are also using a system to summarise consultations and organise the information into a clinical letter format automatically. No personally identifiable data is shared with any third party without your explicit consent. The system involves listening to the audio of the consultation and transcribing it to text. Artificial intelligence is only used to structure the text into a standardised medical format.   • Please arrive for your appointment 5 minutes in advance so that the necessary paperwork can be completed. Late arrival may result in reduced consultation time or forfeiting of the appointment. Adam Bray Dermatology Ltd will endeavour to ensure that your appointment runs to time. However every patient has different needs and we also wish to ensure that each patient has the consultation time they require.  We may also try to fit in additional tests or procedures on the same day as your consultation for your convenience if required. Should we need to cancel or postpone your appointment at short notice, we will make every effort to contact you in advance.

 

REFERRER/GP:

It is usual medical practice for us to inform your GP (and/or other healthcare provider referring you to us) by sending them a letter summarising your consultation and the plan for treatment. If you do not wish this to happen then you must let us know at or immediately after your consultation or treatment.

 

SELF-FUNDING PATIENTS:

You agree to pay for the services that we have provided to you, as per the fee schedule detailed above. For consultations payments should be made in advance by bank transfer – details will be sent. 

 

For procedures, the fees charged by us are exclusive to any additional charges that may be made by the hospital. Procedure fees are payable by direct bank transfer 7 days before your operation date. A cheque may be accepted but this will need to clear 7 days before your operation date.

 

Prices for other tests, surgery or services which are not listed will be quoted by Adam Bray Dermatology Ltd and The Nuffield Hospital Bristol – The Chesterfield in advance, following consultation.

 

INSURED PATIENTS:

We will invoice your insurance company directly if you provide us with complete details of your scheme, policy number and pre-authorisation number. Please be advised that insured patients are liable for full payment as the contract to provide services is with you the patient, and not the insurance company. You must therefore inform your insurance company of consultations and surgery which you organise with us and confirm you have adequate cover. If your insurer declines to settle your invoice in part or in full, then any shortfall or lack of cover will be due from you. If you are unsure we recommend you contact your insurer for advice before scheduling any services with us. We will adhere to the fee schedules set by the major insurance companies and details may be obtained directly from the individual insurance company. These fees differ to the fee schedule which we have for self-funding patients. 

 

SPONSORED PATIENTS:

A sponsored patient is required to settle their account on the day of the appointment, as per the arrangements for a self-funding patient, by debit or credit card. A guarantee of payment or any other payment is not accepted.

 

LATE OR RETURNED PAYMENTS:

Interest on overdue invoices is applied at a rate of 8% a year plus the reference rate and is applicable once 14 days have elapsed following consultation or treatment. Adam Bray Dermatology Ltd reserves the right to charge a £10 administration fee for late or returned payments in addition to reasonable debt recovery costs incurred.

 

UNPAID EXCESSES:

Insured patients will be liable for any excess payment as determined by their private medical insurance contract. This will be clearly communicated by your insurer and is normally paid directly to the provider (Adam Bray Dermatology Ltd). We reserve the right NOT to provide further medical services in the event of any late or outstanding excess payments to us. If you are in the middle of a treatment pathway then onward referral, or referral back to your GP, will be arranged.

 

REFUNDS:

Refunds are not provided.

 

FRAUD AND DEBT COLLECTION:

Information may be disclosed to others, including debt collection agencies, with a view to recovering any unpaid debts or other costs. Where we instruct debt collection agencies, you may become liable for any additional costs.

 

DATA PROTECTION & INFORMATION GOVERNANCE:

Please be aware that any information you send by email is sent 'in the clear' unless specifically encrypted by you. This means the information could be intercepted and read by an unauthorised intermediary. If the information is sensitive then please encrypt your message (your existing email application may offer this capability or use your search engine to find one). If you send an encrypted message you will receive an encrypted reply, otherwise the reply will be sent in the clear. Alternatively please request a telephone call back and leave your contact number.

 

Any personal data that we record during your consultation will be held securely encrypted. We will ask your consent to share any of this data with you by email for your convenience (e.g. a summary of your consultation or copies of photographs). These can be sent via encrypted PDF attached to normal email or via fully encrypted email if you prefer. No personal data will be shared with any other person or body without your explicit consent except for reasons of managing your direct medical care (e.g. your GP or The Nuffield Hospital or onward referral to another specialist by prior agreement with you).

 

ARTIFICIAL INTELLIGENCE FOR DIAGNOSIS AND TREATMENT: 

For the avoidance of doubt, we do not use artificial intelligence for diagnosis or treatment decisions.

 

ARTIFICIAL INTELLIGENCE FOR ADMINISTRATION:

We are using artifical intelligence to streamline administrative tasks. Voice to text digital dictation is used to prepare clinical records and correspondence, to reduce the need for typing. We are also using a system to summarise consultations and organise the information into a clinical letter format automatically. No personally identifiable data is shared with any third party without your explicit consent. The system involves listening to the audio of the consultation and transcribing it to text. Artificial intelligence is only used to structure the text into a standardised medical format.
